SURGICAL TREATMENT OF PORTAL HYPERTENSION
The Editorial Board of “MIA Medical Bulletin” decided to publish the series of articles about important problems of modern surgery, which were included in the Research and Development Plan of the Ministry of the Interior of Russia. This is the first publication that deals with surgical treatment of portal hypertension. In next issues we will focus on open surgery for treatment of portal hypertension, minimal invasive surgery methods using in treatment of this pathology (endoscopic and endovascular surgeries). The final publication will be the article about cell technologies in treatment of this pathology including use of a mesenchymal stem cells-based medicinal product.

Chronic pancreatitis is the inflammatory and degenerative disease characterized by progressive irreversible fibrotic and degenerative changes in a pancreas. The main signs of clinical presentation are a constant pain syndrome and alimentary disorders. Such diseases as pancreatic hypertension, cystic transformation of a pancreas, parenchyma calcification, diabetes are often in this pathology. Typical complications are biliary hypertension, duodenal ileus, extrahepatic portal hypertension.

PREVALENCE OF RISK FACTORS AND DOCTORS’ HEALTH STATUS

175 doctors of the Medical Care Unit of the Ministry of the Interior of Russia in the Rostov Region aged 26 to 73 took part in the study for health assessment subject to gender characteristics and the rate of prevalence of main risk factors of chronic noninfectious diseases. It was detected a high level of prevalence of risk factors of chronic noninfectious diseases among the examined group of doctors.

ACTUAL PRACTICAL USE OF DIAGNOSTICS AND TREATMENT METHODS
OF OBSTRUCTIVE SLEEP APNEA SYNDROME

OF RUSSIA IN THE VOLGOGRAD REGION)

There are 89 sleep disorders and pathological conditions associated with sleep in the International Classification of Diseases. About half of adults occasionally experience problems with sleep. 5% of the adult population suffer from obstructive sleep apnea syndrome (OSAS). Timely treatment of OSAS makes it possible to treat effectively such pathologies as symptomatic hypertension and heart blockades caused by sleep apnea and in the most cases to prevent unwanted sequelae.

DIFFICULTIES OF DIAGNOSIS OF ACUTE AORTIC DISSECTION

The article shows data on the prevalence, classification, diagnostic features of aortic dissection; it is a frequent life-threatening acute disease of a cardiovascular system, which can be fatal in a short time. The authors describe the clinical case of atypical acute general aortic dissection in a young man (a police officer) with a favorable outcome.

DETECTION OF A THYROID GLAND STATUS IN THE MEDICAL EXAMINATION
OF SCHOOL LEAVERS – CANDIDATES FOR STUDY
IN EDUCATIONAL ORGANIZATIONS OF THE MINISTRY OF INTERIOR
OF RUSSIA LIVING IN REGIONS WITH PARTIAL ENVIRONMENTAL

Anthropometric and sonographic examinations of 92 schoolchildren of junior and senior age groups were conducted. Sonographic measurement of a thyroid volume is necessary for the assessment of thyromegaly because the ultrasound detection of a thyroid volume is objective compared to palpation. Assessment of thyroid size should be carried out in accordance with the child’s body area; it is calculated based on his/her height and weight. It is advisable to include US of a thyroid gland in the list of mandatory medical examinations of candidates for study in educational organizations of the Ministry of Interior of Russia living in endemic regions with environmental iodine deficiency.

SOME ASPECTS OF CARE FOR PATIENTS WITH NONALCOHOLIC

Insulin resistance is a pathogenetic basis of nonalcoholic steatohepatiatis associated with heredity. The combination of ursodeoxycholic acid, metforminum and thioctic acid is sufficiently studied, optimal and safe for complex pathogenetic treatment of nonalcoholic steatohepatiatis. Ursodeoxycholic acid and thioctic acid contribute to antioxidant protection, so it can confirms using of this drugs for prevention and treatment of liver steatosis and nonalcoholic steatohepatiatis.

The article deals with the problem of a frequent skin defect striae (traction atrophy). It is widespread among men and women of different ages and causes serious cosmetic problems and emotional stress. Different methods of striae elimination are developed constantly because of lack of a universal method for striae prevention and treatment. The article shows the most effective methods.

CLINICAL AND SOCIAL CHARACTERISTICS OF PATIENTS
WITH MICROBIAL ECZEMA

126 adult patients with microbial eczema were examined. The examination included quality of life assessment, comorbidity analysis, analysis of microbial eczema clinical course, bacteriological examination from niduses. A high level of bacterial colonization was detected. Microbial eczema reduced work capacity and quality of life (Dermatology Life Quality Index (DLQI) in this group of patients was 16.9+, - 1.12 points). It indicates a high degree of negative impact of the disease on quality of life.

CONE-BEAM COMPUTED TOMOGRAPHY IN RHEUMATOLOGY

The high prevalence and significant social and economic consequences of rheumatic diseases cause necessity of the introduction of high-tech diagnostic methods into clinical practice. One of the most promising innovations is cone-beam computed tomography (CBCT). The results of 248 CBCT of hands and feet were analyzed for detecting the severity of the articular syndrome in patients (n=124) with rheumatic diseases (rheumatoid arthritis, deforming osteoarthrosis, psoriatic arthropathy, gouty arthritis). There were higher rates of cone-beam computed tomography when comparing the changes detected by standard radiography and CBCT; the difference was four times or more for most of signs.

Penile fracture is a rare pathology. Diagnosis is made based on complaints, typical clinical picture taking into account the mechanism of injury. Magnetic resonance imaging of a penis is superior to all alternative methods of examination in controversial and ambiguous clinical cases. The indications for operation are penile fracture, hematoma, penile deformation, erectile dysfunction risk. The “gold standard” in treatment of penile fracture is the surgical treatment in maximum short terms: hematoma removal, closure of the defect of the tunica albuginea of the corpus cavernosum, ligation of vessels.

The article deals with the results of psychophysical trainings for officers of internal affairs agencies, which were referred to the Hospital of the Medical Care Unit of the Ministry of the Interior of Russia in the Ivanovo Region for medical and psychological rehabilitation after service in special professional conditions because of fatigue and malaise. The authors analyzed the effect of modern programs of rehabilitation for mobilization of patients’ reserve possibilities.

STRESS INDEXES AND STRESS COPING
STRATEGIES IN POLICE OFFICERS AND RESCUERS

The technique “Stress-PIE” by E. Inanova and D. Amirhan’s Coping Strategy were used for the examination of stress levels and types, stress coping strategies in 816 police officers (798 men and 18 women, n=470) and workers of the MES of Russia (n=346) aged 21 to 52 (the average age – 33.8 Ѓ} 6.8). A middle stress level was detected in 20.3%; a low stress level was detected in 79.7%. The police officers and commanders had higher stress indexes. Emotional stress was more pronounced in women; information stress was more pronounced in workers with diseases. 93% of respondents had problem-oriented stress coping strategies, more often in rescuers (firefighters) and workers with diseases. Intensity of stress was directly related to age, length of service, bodymass index, irrational response strategies (search for social support, avoidance).

COMPLICATIONS OF PROLONGED WEARING OF CONTACT LENSES
Popularization of contact correction of vision and its benefits has determined a superficial approach to the assignment and use of contact lenses. The author of the article describe the complication that arise during contact correction of vision, ways of prevention, as well as sharing his own observation and experience in the treatment of identified complications.

During its 95-year history, medical service of the internal affairs agencies has undergone many transformations and reorganizations. The service passed this way with dignity, not only retained material resources, people and traditions, but also developed and increased the potential of the departmental health service.
The article deals with the main factors motivating high quality and efficient work of doctors and nursing staff of radiodiagnostics units. The Sh. Ritchie and P. Martin method of motivation testing and modeling was used for the study; this method distinguishes 12 main factors of motivation. The survey results of 56 employees of radiodiagnostics units were analyzed for detecting motivation factors. The heads of the Medical Service constantly improve the organization of military medical expertise since the forming departmental health service in internal affairs agencies of the Russian Federation. This creates a reliable instrument for medical selection of citizens in law-enforcement authorities, that these persons will be able to perform official duties most effectively and without health detriment.
At present, successful operation of the departmental health system is not possible without the regular improvement and adoption of new management technologies. Only the modern management system, which bases on ever-changing conditions of medical support, can maintain its stability and pave the way for further development.
