The Main Clinical Hospital of the MIA of Russia had the seventy-fifth anniversary in May 2017. The history of the Hospital is closely connected with the great and tragic events in the history of our country. At present the Hospital is a modern multifunctional health institution, the leader of the departmental medical service. The main priority of the Main Clinical Hospital of the MIA of Russia is high quality of treatment and advertence to patients.

The article deals with the problems of diagnosis and surgical treatment of postsurgical biliary peritonitis. The authors focus on development of the rational sequence of preventive, diagnostic and therapeutic measures for patients with postsurgical biliary peritonitis after operational interventions in a biliopancreatoduodenal zone.

HEART DISEASE IN THE PATIENT WITH GENERALIZED SARCOIDOSIS

Diagnostics of extrapulmonary manifestations of sarcoidosis is based primarily on a thorough assessment of the dynamics of clinical manifestations of the disease and markers of its activity. Usually extrapulmonary manifestations of sarcoidosis require using both glucocorticosteroids and new methods of pathogenetic therapy. The clinical studies of patients with pulmonary sarcoidosis and heart disease may be useful to clarify the mechanisms of progression of sarcoidosis as a systemic disease.

PNEUMONIA IN PATIENTS WITH IMMUNODEFICIENCY

At present detecting pneumonia with concomitant decrease in immunity is growing constantly. Doctors of various specialties are increasingly faced with the problem of diagnosis and treatment of pneumonia in patients with immunodeficiency. It is necessary to imagine clearly all the variants of the X-ray picture of infiltrative changes in the lungs. Immunocorrecting therapy is most effective only when the level of immunity is detected and treatment is directed at a specific cause of the disease.

DIABETES IN POLICE OFFICERS: EPIDEMIOLOGY, CLINICAL FEATURES AND JOB PERFORMANCE

Diabetes is an urgent problem of modern medicine; it connects with disease incidence and the tendency to growth, clinical features, quite often progredient clinical course and complications including life-threatening. This disease requires lifelong medical support, adequate secondary prevention, and systematic quality control of job performance inside of individual medical and psychological monitoring. The article shows the actual prevalence of type 2 diabetes among police officers, variants of clinical course, disabilities, and areas of prevention.

FROM THE HISTORY OF TUBERCULOSIS TREATMENT IN THE TATARIAN ASSR IN THE LATE 1940s

The article was made with use of materials of the diary “The black days of consumptive life”. The authors consider ego-documents as a source on the history of medicine and social history. Diary notes contain not only some disease experience, but also the facts of postwar everyday life. Analyzing the mortality rates of the population in the late 1940s, the authors detect the place of tuberculosis among the causes of death in Tatarian ASSR.

DIAGNOSTIC STANDARDS OF RADIODIAGNOSTICS
Based on the departmental standard OST PVB 91500.09.0001-1999 the standard protocols (algorithms) of the examination of patients with the main clinical syndromes have been developed and tested including acute abdomen, renal colic, diffuse pulmonary lesion, head trauma, acute cerebral insult, obstructive jaundice, polyarticular lesion, etc. Using of algorithms of patients’ examination led to significant reduction in the terms of the examination (on average by 53.4%) and reducing the cost of diagnosis (on average 30.3%). The introduction of patient examination protocols made it possible to increase diagnostic
accuracy (by 24.6%), sensitivity (by 17.3%), specificity (by 12.1%).

RECONSTRUCTIVE SURGERY IN URETERAL STRICTURES AND OBLITERATIONS

Recovery patency of the upper urinary tract in ureteral extensive injuries and strictures remains one of the most difficult problems of urologic surgery. Postoperative iatrogenic complications, radiation treatment, tuberculosis, schistosomiasis, retroperitoneal fibrosis, gunshot wounds, radical surgery for malignant neoplasms, using endoscopic and endovideosurgical methods of treatment can cause significant ureteral defects including extensive strictures and urinary fistulas. Impossibility of restoring ureteral patency with the use of unchanged tissues of the urinary tract often causes nephrectomy or using persistent urinary drainages.

SPECIFICS OF DIAGNOSTICS OF MALIGNANT NEOPLASMS OF NOSE AND PARANASAL SINUSES IN A POLYCLINIC

The use of maximally informative research methods is necessary for diagnostics of malignant diseases of ENT organs, in particular malignant neoplasms of nose and paranasal sinuses. These methods are multispiral computed tomography, magnetic resonance imaging and optical endoscopy. The article shows the case study when the efficient algorithm of actions in a polyclinic makes it possible to render the most effective medical aid to the patient at other stages of treatment and has a positive effect on its results.

SPECIFICS OF MEDICAL EXAMINATION OF PERSONS WITH REFRACTIVE ERRORS

The article deals with the detecting specifics of medical examination of persons with refractive errors. Knowledge of examination specifics will enable to avoid possible diagnostic errors and formulate the correct conclusion on medical classification for military service in candidates for service in internal affairs agencies and educational organizations of the Ministry of Interior of Russia.

MELANOMA OF CHOROID

Uveal melanoma is the most common primary malignant tumor in age of 30–80. The article shows the case study of the patient M., age of 66, with a diagnosis of melanoma of choroid T4N0M0 in the left eye and subsequent enucleation of the left eye. This case study demonstrates the importance of timely and complete examination of patients in the course of clinical examination in outpatient clinic practice.

Some noninfectious diseases are caused by the intensity and characteristics of the working process, professional stress and accompanied by psychic and emotional tension. Sleep disorders occupy a special place among psychosomatic disorders including adaptation and emotional disorders associated with professional specificity. Sleep disorders often accompany and increase forming separate common noninfectious diseases.

During its 95-year history, medical service of the internal affairs agencies has undergone many transformations and reorganizations. The service passed this way with dignity, not only retained material resources, people and traditions, but also developed and increased the potential of the departmental health service.
The article deals with the main factors motivating high quality and efficient work of doctors and nursing staff of radiodiagnostics units. The Sh. Ritchie and P. Martin method of motivation testing and modeling was used for the study; this method distinguishes 12 main factors of motivation. The survey results of 56 employees of radiodiagnostics units were analyzed for detecting motivation factors.
