CLINICAL USE OF PREVENTION METHODS OF VENOUS THROMBOEMBOLIC COMPLICATIONS IN TRAUMATOLOGICAL PATIENTS

Timely detection of deep venous thrombosis and prophylaxis thromboembolic complications in traumatological patients is a very important medical problem. Consolidated efforts of doctors of all specialties are necessary to solve this problem. The article deals with the analysis of results of clinical use of modern prevention methods of venous thromboembolic complications in 109 patients with orthopedic injuries. Preventive measures included the use of mechanical methods, pharmacological prophylaxis with heparins of various molecular weights. Endovascular interventions with the installation of a cava filter were performed in the case of embolus thrombosis for the prevention of pulmonary embolism.

RIGHT VENTRICLE INFARCTION IN INTERNAL DISEASE CLINIC

The article deals with the analysis of publications about the specifics of diagnosis and treatment of right ventricle infarction. The distinctions of isolated right ventricle infarction are the absence of symptoms of acute left heart strain and blood stagnation of pulmonary circulation. Injections of vasodilators and diuretics, stress tests, inotropic support are limited in treatment of isolated right ventricle infarction. Closer monitoring of cardiac rhythm and cardiac rate is necessary. Knowledge of pathogenesis, clinical manifestations and therapeutic approaches to right ventricle infarction is an important element of the professional qualification of a modern doctor.

RATIONALE FOR EARLY CLINICAL AND INSTRUMENTAL DIAGNOSTICS OF HYPERTENSION STAGE 1 IN PILOT PERSONNEL
The article presents data characterizing the prevalence of hypertension among the pilot personnel, the recommendations of the ANA, ACC 2017, offering new criteria for diagnosis of hypertension, one of the available methods for evaluating endothelial dysfunction (ED) recommended by the author for the diagnosis of hypertension Stage 1 in pilot personnel. The results of the investigation of pulse wave velocity and ED in flying professionals with certain severity of pathology are presented too.

POLYPRAGMASY: ASPECTS, PROBLEMS AND SOLUTIONS

The problem of polypragmasy is closely related to the problem of drug interaction. Polypragmasy makes it impossible to control the effectiveness of therapy, increases material costs, and reduces compliance. In this connection, it is necessary to develop and implement effective mechanisms of correct prescription of medicinal products only in circumstances of appropriate therapeutic indications, patients’ awareness about the benefits of the preferred therapeutic approach and its possible complications in treatment, and regular monitoring of patients’ compliance. Recommended criteria for evaluating the rationality of drug therapy (STOPP/START criteria, Beers Criteria, Index MAI) should be used for effective treatment and minimization of drug adverse side effects. Medical professionals should try to balance individual risks and benefits for patients using all possible ways and implement appropriate mechanisms for monitoring and analyzing decisions about prescribing medicines to patients, especially from highrisk groups.

IRRITABLE BOWEL SYNDROME – ROMAN CRITERIA IV (2016)

The article deals with one of the most common functional gastrointestinal diseases, irritable bowel syndrome (IBS). The authors give the new diagnostic criteria, detect the main differences from the previous version, and describe the specifics of pathophysiology, diagnosis of IBS. The article presents a modern view of pharmacotherapy of IBS according to the Roman criteria IV.

A CASE STUDY OF MASSIVE MIDDLE EAR CHOLESTEATOMA

The article deals with a rare case study of massive middle ear cholesteatoma in a man of age 24. A mass lesion with clear contours and visualized auditory ossicles was detected by the use of computed tomography. It was removed by a sanitizing operation with ossiculoplasty and tympanoplasty.

EXPERIENCE OF USING INTEGRATED REHABILITATION SYSTEM SPA CAPSULE “ALPHA LED LIGHT SPA™” IN THE SANATORIUM “RUSSIA” OF THE NATIONAL GUARD FORCES COMMAND OF THE RUSSIAN FEDERATION

The article deals with the analysis of results of clinical and psychopathological examination of 53 military persons, which underwent rehabilitation after combat missions in the sanatorium “Russia” of the National Guard Forces Command of the Russian Federation. The integrated rehabilitation system Spa Capsule “Alpha Led Light Spa™” from “Sybaritic” was used in rehabilitation programs developed in the sanatorium. The purpose of the research was to study the effectiveness of this system for medical and psychological rehabilitation. Pathopsychic register-syndromes were detected in patients in the process of research. More active positive dynamics of the course of psychopathological disorders was in the research group compared with the control group after using the integrated rehabilitation system Spa Capsule “Alpha Led Light Spa™” from “Sybaritic”.

There is no doubt that, it will increase combat effectiveness in the performance of service tasks.

PRINCIPLES OF SELECTION IN GROUPS OF HIGH RISK OF PRE-NEUROTIC DISORDERS AMONG POLICE OFFICERS

The article deals with the main psychoprophylactic approaches to the formation of a risk group in the progress of pre-neurotic disorders among police officers. The authors give an algorithm for evaluation and early detection of personality characteristics, which can be a prerequisite for forming pre-neurotic disorders.

The authors studied in comparison healing of skin wounds of the same length and depth given laboratory animals (rats) in the experiment with lasers, radio-wave emission, electrosurgical exposure, plasma and fluid streams.
