THE PROFESSIONAL STANDARD OF AN EXPERT ON MILITARY MEDICAL EXAMINATION

The article deals with military medical examination as a type of medical examination, which has a significant specificity; it is demonstrated from a professional point of view. Its methods and competences of an expert on military medical examination are detected. The authors performed the comparative analysis of qualification characteristics of medical specialists of military medical commissions in order to identify their compliance with the goals and objectives of military medical examination. The results of the analysis made it possible to justify the reasonability of introducing the professional standard for an expert on military medical examination.

THE ROLE OF NUTRITION IN TREATMENT AND PREVENTION OF CHRONIC NON-COMMUNICABLE DISEASES

At present, there is no consistent approach to the treatment of patients with chronic non-communicable diseases in healthcare in Russia. There is no adequate assessment of the nutritional status and structure of rations in integrated staged rehabilitation and prevention programs in our country, whereas clinical practice guidelines that cover various aspects of this multidisciplinary problem are already used abroad.

SELF-EXPANDING NITINOL STENTS IN MULTIMODALITY THERAPY OF VARICEAL ESOPHAGEAL HEMORRHAGE

The article shows the results of multimodality therapy of 17 patients with portal hypertension and esophageal hemorrhage. The implantation of a self-expanding nitinol stent for hemostasis in the area of esophageal bleeding was carried out as an alternative to traditional balloon tamponade with a Sengstaken – Blakemore tube.

SURGICAL PATIENT: NUTRITION BEFORE SURGERY

Over the past two decades, planned surgical care has been provided to patients according to the concept of the Enhanced Recovery Program. Nutritional support for surgical patients is an important part of the Program. Correction of nutritional status should begin at the prehospital stage after detecting indications for surgical treatment. It is necessary to refuse traditional fasting and prescribe a carbohydrate drink before planned surgical operations according to the recommendations of specialists. Establishment of “carbohydrate comfort” leads to decreasing insulin resistance and stressful hyperglycemia in the postoperative period, and this helps reduce the number of complications and shorten hospital stay.

DUAL-ENERGY COMPUTED TOMOGRAPHY IN THE DIAGNOSTICS OF UROLITHIASIS

Urolithiasis is one of the most common urological diseases that occurs in 3% of population at least, and most often in people of working age (30–50 years old). The development and improvement of new diagnostic methods is of great social importance because of the relevance of the disease. Radiological imaging methods play a key role in the diagnosis of urolithiasis. At present using dual-energy computed tomography in urological practice is widely discussed. The study of one of its main potentialities (detecting the composition of urinary calculuses) demonstrated reliable diagnostic value. The article shows the results of dual energy computed tomography application in patients with urolithiasis.

GENERAL MAGNETOTHERAPY IN THE INTEGRATED TREATMENT OF ACUTE ENDOMETRITIS AFTER INEVITABLE MISCARRIAGE IN THE FIRST TRIMESTER

The article deals with the treatment of patients with acute endometritis after inevitable miscarriage in the first trimester. Early inclusion of general magnetotherapy in a comprehensive rehabilitation program increases the effectiveness of treatment and rehabilitation measures, it has a positive effect on clinical manifestations and contributes to the earlier and significant correction of abnormalities of laboratory and functional parameters. This method is an effective way to prevent reproductive disorders in patients with this pathology too. Assistant of the department E. Fedorova took an active part in the work on the article.

Obesity is a chronic polyetiologic disease characterized by excessive accumulation of adipose tissue in the body. It leads to deterioration of life quality and reducing overall life span due to the frequent progress of severe concomitant diseases. Exogenic constitutive obesity is detected in adolescence in most cases (more than 90%).

The authors present the experience of using minimally invasive techniques for osteosynthesis of acetabular and pelvic fractures in 84 patients in 2003–2016. The age of patients ranged from 17 to 70 years. The authors used cannulated screws and threaded wires for osteosynthesis. Indications for the use of minimally invasive osteosynthesis of pelvic fractures have been detected. Road traffic accidents cause great social, material and demographic damage to the Russian economy and society. Inclusion of obstructive sleep apnea syndrome in the list of diseases that may be grounds for limiting driver’s licenses will reduce road mortality by 20%. This justifies the need for adjustments of the current legal acts on compulsory medical examination of drivers.
Health care and social protection of the population are the most urgent problems, especially in the context of the recently changed social, economic and political situation in the country.
