DEVELOPMENT OF DEPARTMENTAL MEDICINE AS A BASE OF HEALTH
PROMOTION OF INTERNAL AFFAIRS AGENCIES STAFF

At present, the Office of Medical Support of the Department of Logistical and Medical Support of the Ministry of Internal Affairs of the Russian Federation manages the Medical Support Service of the Ministry. The Office
of Medical Support conducts organizational and methodological management of the Central Medical Care Unit of the Ministry of Internal Affairs of Russia, 84 Medical Care Units of the Ministry of Internal Affairs of Russia in subjects of the Russian Federation and 41 health institutions of the Ministry. As a result of reforming the Ministry, in 2016, six sanatoria of abolished federal agencies and the Central Polyclinic of the FDCS of Russia (renamed to the Central Polyclinic N 3 of the Ministry of Internal Affairs of Russia) were transferred under the supervision of the Office of Medical Support. Therefore, the number of personnel of the Medical Support Service of the Ministry of Internal Affairs of Russia has increased and is about 34 thousand people now.

ENDOVASCULAR SURGICAL CORRECTION OF PORTAL HYPERTENSION

Transjugular intrahepatic portosystemic shunt (TIPS) is the most commonly used method of portal hypertension operative repair. Endovascular embolization of stomach varicosities and a splenic artery is used much less often.

PHAGOPROPHYLAXIS AND BACTERIOPHAGE TREATMENT
OF SURGICAL INFECTIONS

Search for alternative and synergistic of antibiotics methods for the prevention and treatment of infectious complications of wounds is a topical problem due to the constant formation of antibiotic resistance of pathogenic microorganisms. The article deals with the various aspects of bacteriophages using for these purposes. There are biological properties of bacteriophages and the historical background on these preparations in the article. The authors assessed the possibility of phage drugs for treatment and prevention of infectious complications of wounds and detected that the preparations containing several active phages against the main pathogens of surgical infection were most suitable for these purposes. The authors assessed the polyvalent pyobacteriophage “Sextaphag” in vitro and its efficacy in clinical use. Based on the analysis of literature data and results of their own studies the authors concluded that the use of pyobacteriophages was promising for prevention and treatment of infectious complications of wounds.

The article shows the efficacy of silica enterosorbent “White Carbon” in the complex therapy of epatobiliary system’s diseases, such as toxic and viral hepatitises, chronic non-calculous cholecystitis, nonalcoholic fatty liver disease.

PNEUMONIA IN PATIENTS WITH IMMUNODEFICIENCY

At present, detecting pneumonia with concomitant decrease in immunity is growing constantly. Doctors of various specialties are increasingly faced with the problem of diagnosis and treatment of pneumonia in patients with immunodeficiency. It is necessary to imagine clearly all the variants of the X-ray picture of infiltrative changes in the lungs. Immunocorrecting therapy is most effective only when the level of immunity is detected and treatment is directed at a specific cause of the disease.

HYBRID REGIMEN OF ERADICATION THERAPY OF HELICOBACTER PYLORI
INFECTION IN TERMS OF “THE MAASTRICHT V CONSENSUS REPORT”

Clinicians observe a marked decrease in the effectiveness of empirical therapy of Helicobacter pylori (H.pylori) infection due to the increasing resistance to antibiotics. In view of this fact, it is very important to detect the regimens of eradication therapy, which have acceptable effectiveness, safety and tolerance now. The hybrid eradication therapy regimen of P.I. Hsu, et al (2011) attracted the wide attention of the gastroenterological community due to its high efficiency and good safety profile. Aspects of hybrid therapy are described insufficiently in the Russian medical literature, so the authors consider it expedient to systematize and present data on this regimen of eradication therapy.

OPTIMIZATION OF MEDICAL EXAMINATION OF ATTESTED OFFICERS

The authors analyze the applicable documents regulating the work of doctors of military physician boards for medical examination of officers of internal affairs agencies with arterial hypertension; they point to some current contradictions in these documents and suggest a way of their eliminating.

PROBLEMS OF MILITARY MEDICAL EXAMINATION OF HEARING LOSS
The articles deals with the comparative analysis of types and methods of audiologic examination as applied to medical examination of hearing loss. The author gives a brief description of the expert algorithm for military medical examination in various powerwielding agencies of the Russian Federation and analyses the similar algorithms of civilian ministries and agencies, which carry out forensic medical examination, aviation physical examination, occupational pathology and occupation diseases examination. The most urgent problems of military-medical examination of this pathology are detected in the article.

OTOLITHIASIS: THE CLINICAL PROBLEM FOR A THERAPEUTIST (CASE REPORT)

The article deals with the diagnosis and therapy of one of the rarely diagnosed pathologies otolithiasis (canalolithiasis) as the most common cause of benign paroxysmal positional vertigo. If interpretation of the symptomatology and direction of the diagnostic search are correct this disease is well diagnosed and cured, using simple methods performed in the outpatient setting in most cases.

Therapeutic alternatives of benign prostatic hyperplasia are widely used to treat patients with aggravated history. These methods do not cause surgical trauma, do not require anesthesia; therefore, they can be used in the outpatient setting.

During its 95-year history, medical service of the internal affairs agencies has undergone many transformations and reorganizations. The service passed this way with dignity, not only retained material resources, people and traditions, but also developed and increased the potential of the departmental health service.
The article deals with the main factors motivating high quality and efficient work of doctors and nursing staff of radiodiagnostics units. The Sh. Ritchie and P. Martin method of motivation testing and modeling was used for the study; this method distinguishes 12 main factors of motivation. The survey results of 56 employees of radiodiagnostics units were analyzed for detecting motivation factors. The heads of the Medical Service constantly improve the organization of military medical expertise since the forming departmental health service in internal affairs agencies of the Russian Federation. This creates a reliable instrument for medical selection of citizens in law-enforcement authorities, that these persons will be able to perform official duties most effectively and without health detriment.
